How to Apply

Application Deadline*
To apply for a specific semester, application materials must be received by the following dates:
• Fall Semester: July 1
• Spring: November 1
• Summer: April 1

Fall, Spring or Summer

*Class sizes are limited and qualified applicants are accepted on a first-come basis.

STEP 1 - Contact people and agencies required to provide information on behalf of the applicant to the Department of Physical Therapy and Rehabilitation Science.

  1. Arrange to have one official copy of transcripts from each college and university that you have attended sent directly to the department. Please include both undergraduate and graduate course work.

  2. Request a letter of recommendation from three (3) people. Two letters from currently practicing physical therapists are recommended. Distribute the forms, giving the individuals who are writing the recommendations enough notice to thoughtfully complete the form before you plan to
    send in your application. Instruct them that after they have completed the recommendation, they should put it in an envelope, seal the envelope, sign over the seal, then return the sealed envelope to you.

STEP 2 - Complete all required application forms and documentation and mail in one package to the Department of Physical Therapy and Rehabilitation Science.
Use the application packet to the right. We suggest you complete the forms by typing into the fields using your computer before printing them out . This will avoid delays caused by illegible hand-written applications. Adobe Acrobat Reader is required to view and print the forms.
