STEP 1 - Contact agencies required to send information directly to the Department of Physical Therapy and Rehabilitation Science.

  1. Arrange to take the Graduate Record Examination (GRE) General Test. Your application cannot be completely processed until we have received your scores. Use institution code 6895 and department code 0619 to have an official copy of your scores mailed directly to our office.

  2. Arrange to have one official copy of transcripts from each college and university that you have attended sent directly to the department.

  3. If you will take prerequisite courses in the Fall semester prior to the year you would begin the DPT program, send us an additional transcript after course work is complete (it can be unofficial if you still have courses to take in the Spring before the DPT program start date).

  4. If not available at the time of application, official transcripts must be sent to the department at the end of the following Spring semester indicating an undergraduate degree has been achieved.

STEP 2 - Complete all required application forms and documentation and mail in one package to the Department of Physical Therapy and Rehabilitation Science.
Use the application packet to the right. We suggest you complete the forms by typing into the fields using your computer before printing them out . This will avoid delays caused by illegible hand-written applications. Adobe Acrobat Reader is required to view and print the forms.

International Students
You are considered an international student if you require a visa, or are currently reside in the U.S. with non-immigrant status, or currently residing in the U.S. and applying for permanent resident. International students may require additional admission documentation, test scores and/or course work evaluation.

Students with degrees from a university outside the U.S. may be considered provided they meet the requirements equivalent to a bachelor's degree from a program in the U.S.

If English is a second language, applicants must take the Test of English as a Foreign Language (TOEFL), and arrange for scores to be sent directly to the KU Department of Physical Therapy and Rehabilitation Science. A satisfactory score on the TOEFL must be obtained within the previous two (2) years.
